After activating your benefit, download your confirmation letter right from this page. Tell your in-network provider you want to use your EAP benefits and give them the letter. It can help them submit your claim.

Payment for provider services is subject to eligibility for employee assistance program (EAP) services at the time services are provided, benefit plan limitations, availability of remaining coverage, and any limitations imposed by regulatory requirements.
